Do not submit:
- Patient names

Information Handled During Client Services
Website inquiries and contracted Ellery Health Partners services are separate.
Submitting a practice inquiry does not authorize Ellery Health Partners to access PHI and does not establish a healthcare-services relationship.
Before Ellery Health Partners accesses PHI on behalf of a healthcare organization:
- The applicable client agreement must be executed.
- A Business Associate Agreement (BAA) must be executed where required.
- The client must provision appropriate access.
- Applicable security, access, and onboarding requirements must be completed.
Whenever operationally feasible, Ellery Health Partners personnel perform contracted payer-administration work inside systems controlled or approved by the client, such as:
- The client’s EHR
- Practice-management systems
- Availity
- Payer portals
- Benefit-management portals
- Client-approved secure document environments
- Client-approved secure fax systems
- Authorized payer telephone workflows
The systems used depend on the contracted service; not every service uses every system.
Ellery Health Partners does not intend to create or maintain a separate public-facing patient database through elleryhealthpartners.com.
If patient information must be transferred outside a client system in connection with contracted services, the approved transfer method and applicable safeguards will be established through the client’s onboarding process and governing agreements.
PHI handled during contracted services is governed by the applicable BAA, service agreement, security requirements, and applicable law rather than by the public practice inquiry form.
